When is the training? The Transitional Care Coordination training is a multi-day virtual training. The training will occur on October 28 and 29, 2020. On each day, the training will run from 11:00 a.m. -5:00 p.m. Eastern Time.  All Training Activities will be Conducted via Zoom.

Why should I attend this training?  To end the HIV epidemic, stakeholders throughout the country will need to employ effective, evidence-informed approaches to improve health outcomes across the HIV care continuum. One such approach is the integration of the Transitional Care Coordination (TCC) intervention. The TCC intervention engages people with HIV in the jail setting to support linkage to community-based HIV primary care after incarceration and address client-specific barriers to care, such as housing, transportation, substance use, and mental health. 

This TCC training will be interactive and focused on skills-building for direct service and administrative staff who seek to implement a new TCC program or strengthen existing services for people with HIV who are released from jail settings. The institute seeks to achieve the following goals:

1) define the core components of the Transitional Care Coordination intervention
2) discuss strategies for successfully building relationships with their local jail systems and the importance of working with people with HIV while incarcerated as well as upon release
3) plan for how training participants will implement the Transitional Care Coordination or components of the intervention in their organization/jurisdiction.

To achieve these goals, facilitators will share evidence-informed models and free tools that can be incorporated into ending the HIV epidemic plans or other work to improve outcomes across the HIV care continuum at a local level. Click here to review a draft agenda for the training.
